Oregon Tech Integrated Student Health Center is required by law* to ensure that all students have received two Measles vaccinations. Oregon College Immunization Law (pdf)
Newly admitted students must complete the following health requirements (note: International students must complete a similar process prior to coming to campus; work with your Admissions Counselor to provide the required information). Failure to complete these requirements will result in a “Health Hold” on the student’s account, affecting a student’s ability to conduct Business Office transactions, to register for additional courses, or make changes to their current schedule.
Student Health Portal
An electronic portal is now available for students to complete the two requirements (see below). Access to the portal can be found through the student's Techweb account (click on the "Student" tab, then on the "My Health" icon, then "My Health Portal", then "Forms" to complete the requirements). Step-by-step instructions can be downloaded in the "File Downloads" section on this page. Students are no longer required to provide documentation to support their immunization history, unless the dates they provide for immunizations are inaccurate; in that case, the student must provide documentation so that ISHC staff can confirm the information and then make the corrections in the system.
Requirement 1: Health History
All incoming students are required to complete a Health History Form which provides information about their medical history. This provides ISHC with a more thorough understanding of students' needs in order to enable more targeted services and programming.
Requirement 2: Measles Vaccine Information
Two doses of Measles vaccine (or Measles-Mumps-Rubella, MMR) are required for all full-time college students in Oregon born on or after Jan. 1, 1957. The first dose must be given after the first birthday, and the second dose must have been administered after 1989 and at least 30 days after your first birthday. The dates that the immunizations were administered will be entered into the Student Health Portal. A student's immunization history can often be found:
- In their current doctor's office or medical clinic
- At the Public Health Department
- On their personal immunization card (signed by a medical provider or nurse)
- On their previous college immunization records
- On their high school transcript
- State Immunization Registry

Students who were born in California or Oregon can request their immunization records from the state public health departments here: https://myvaccinerecord.cdph.ca.gov/ (California Digital Vaccine Records) or Oregon Alert System Other states are likely to have similar systems in place.
As indicated above, it is crucial that you enter accurate vaccine dates. If the dates are inaccurate, then you will be required to provide documentation of the immunizations to ISHC staff in order for them to verify the dates and correct the error in the system.
If you are unable to locate your immunization information, ISHC does offer MMR vaccinations for a fee. It will not harm you to have additional immunizations if you have had them before, but they do have to be administered one month apart. Call 541-885-1800 if you need to schedule an immunization.
Exemptions
Students may be exempted from the immunization requirements only if: 1) there is a documented medical condition preventing the immunization, or 2) the student has filed a Nonmedical Vaccine Exemption with ISHC. For medical exemptions, a signed statement from your health care provider is required; for your convenience, we have a form that your provider can complete on our Forms page. To file a Nonmedical Vaccine Exemption, the student must view the required Oregon Department of Public Health video and provided a signed Certificate of Completion to ISHC.
Please be aware that students who are granted an exemption are likely to be prohibited from campus by the Department of Public Health in the event of an outbreak and may not receive a financial refund or additional time to make up any missed coursework. Recommended Immunizations
Although the following immunizations are not required by the state of Oregon for admission, ISHC does recommend that you consider getting immunized against the following diseases:
Tetanus
Td is a tetanus-diphtheria vaccine given to adolescents and adults as a booster shot every 10 years, or after an exposure to tetanus under some circumstances. Tdap is similar to Td but also containing protection against pertussis. A single dose of Tdap is recommended for adolescents 11 or 12 years of age, or in place of one Td booster in older adolescents and adults age 19 through 64.
Meningitis
Consistent with the American College Health Association (ACHA) recommendations, the ISHC recommends that in-coming students discuss vaccination for meningitis with their medical provider, contact their local public health agency, or inquire at the ISCH. Hepatitis A
A serious liver disease caused by the hepatitis a virus (HAV). HAV is usually spread by close personal contact. It can also be spread by eating food or drinking water containing HAV.
- Two doses of the vaccine are needed for lasting protection. These doses should be given at least 6 months apart.
Hepatitis B
A serious liver infection caused by the hepatitis B virus (HBV). For some people, hepatitis B infection becomes chronic, leading to liver failure, liver cancer, or cirrhosis-a condition that causes permanent scarring of the liver.
- Hepatitis B vaccine is usually given as a series of 3 or 4 shots. These doses gives long-term protection from HBV infection, possibly lifelong.
Hepatitis A/B
A combined vaccination usually given as a series of 3 or 4 shots.
