Report Academic Dishonesty
Academic dishonesty is any type of cheating, plagiarism, fabrication or falsification of data, deception of an instructor by providing false information for missing an exam or deadline, or claiming to have submitted coursework when you know you have not.
Academic Dishonesty is defined as cheating, plagiarism or otherwise obtaining grades under false pretenses. Violations include cheating, plagiarism, unauthorized collaboration and fraudulent alteration of academic materials. Read the full policy here. Academic dishonesty can be reported by any Oregon Tech community member.
Report Student Conduct Issue
Disruptive behavior, in or out of a classroom setting, that inhibits a faculty/staff member's ability to conduct class or limits another student's ability to fully access the educational benefits the university has to offer is prohibited. Both students and employees can report disruptive behavior.
Report Campus Hazing
Oregon Tech takes every report of hazing seriously. Hazing is any action or activity that causes or intends to cause physical or mental discomfort or distress, that may demean, degrade or disgrace any person, regardless of location, intent or consent of participants, for the purpose of initiation, admission into, affiliation with, or as a condition for continued membership in a group or organization (on or off campus). Use this form to report campus hazing. All the information you report will be kept confidential.
Report Disruptive Behavior
Oregon Tech expects all members of the campus community (faculty, staff and students) to strive to foster academic excellence and integrity, both in and out of the classroom. The University reserves the right to take necessary and appropriate action to protect the safety and well-being of the campus community. Example: Classroom disruption is behavior that a reasonable person would view as substantially or repeatedly interfering with the instructor’s ability to teach the class or the ability of other students to benefit from the instructional program. Disruptive behavior can be reported by any Oregon Tech community member.

Academic Support Request
Inspire Alerts are for Academic-related issues. Choose High Alert if the student is in immediate danger of failure or other serious consequences. Choose Medium Alert to flag a concern that you feel might result in serious consequences if left unresolved. Give a Commendation to recognize exceptional performance.
Medical Withdrawal
A Medical Withdrawal is requested when a student is suffering from a medical condition that prevents the completion of the term. Medical withdrawals can be for one or more courses the student is registered in for a given term. A member of the Dean of Students team will provide support in processing this request.
